Required Forms

Every camper is required to complete a "Release and Medical Authorization" form.  Please print the form, complete it and either bring completed form to camp check in, or mail to:

Registration is not complete and you will not be allowed to participate until we have this form.  NOTE:  If you are registering for both camps, you only need to complete the form ONCE.

If you are registering online, a form will automatically be emailed to you.
